Application for Child (Under 18)

  2. Ask Social Security office if your child is within the allowable limits considering your income or resources and if you can proceed with the Supplemental Security Income application.
  3. Fill out Child Disability Report online. Part of this report asks for your permission that the child’s doctor can give information regarding the condition of your child. This will help them decide on the claim or benefits that will be given.
  4. Note that you can use your re-entry number to fill out the Child Disability Report online if you were not able to finish accomplishing the form previously.

Application for Adult (Age 18 and above)

  2. Fill out the application form for Social Security Benefits online. But if you have never worked before, skip this step and contact the nearest Social Security office after accomplishing step 3.
